ResiHealth at Hilltop Village

Jane Rupp, FNP-C, is a board-certified Family Nurse Practitioner with more than 15 years of nursing experience, including 8 years in primary care. She provides comprehensive primary care at ResiHealth in Cleveland, Ohio, with particular expertise in the management of hypertension and diabetes. Jane began her career specializing in surgical oncology at University Hospitals, with focused experience in ENT, colorectal, and urology care, where she developed a strong foundation in complex patient management. Before joining ResiHealth, she spent 7 years serving patients and families in Cleveland's Fairfax community at Otis Moss Jr. Health and Community Center, building lasting relationships while managing both acute and chronic conditions. Jane is passionate about creating individualized care plans tailored to each patient's unique goals and lifestyle. She values collaboration, education, and preventive care, and looks forward to partnering with patients to help optimize their health and overall well-being. She is currently accepting new patients at ResiHealth.

Hilltop Village is an affordable housing community serving older adults. ResiHealth is co-located within the building so residents can access care without leaving home. We also host health events and screenings on site.
